I used a 6 x 6 piece of paper and scored at 2 and 4 both ways and then cut up to the score line on opposite sides. 

I used 4 strips of paper cut 5 1/2 x 3/4, 1 strip of paper cut 5 1/2 x 1/2, and 1 cut 7 1/4 x 1/2 (white).
